This allows us to provide the highest quality care without restrictions from insurance companies. Full payment is due at the time of service, but we’re here to make the insurance process easier for you. As a courtesy, we will:
We encourage you to review your benefits to understand your coverage. After 90 days, regardless of claim status, the patient is responsible for any unpaid balance.

We take pride in delivering high-quality, long-lasting restorations. However, restorations may be affected by normal wear, bite forces, or oral health conditions. If a restoration needs replacement or adjustment:
The dentist may extend the warranty in certain cases. Regular maintenance checkups and radiographs are essential for protecting your investment and supporting the longevity of your dental work.
Please provide 24 hours’ notice to cancel or reschedule. A $50 fee applies to late cancellations or missed appointments. Repeated no-shows may result in dismissal from our practice.

By signing, I acknowledge that Del Mar Dental Studio is out of network, and I am responsible for fullpayment. While the practice assists with insurance claims, reimbursement is not guaranteed. Iaccept responsibility for treatment costs, lab fees, and restoration redos after 12 months, and I agreeto the policies on missed appointments, balances, and collections.
